Won't unlock achievements?! Throwing axe bug

I attached an image of my progression of the 3 achievements regarding level and the bar is full yet it is still locked. Why is this?
Also there is a bug with the throwing axes (not sure about the knives). When you have it equipped and get hit with a throwable weapon you cannot throw yours unless you switch to your primary or secondary and back. Not sure if anyone else is running into this problem as well.

It’s even worse, you actually lose the axe as if you had thrown it!
So you are flinched for a second or more and you lose the weapon.

Just yesterday I had it twice in a row… hugely annoying as I lost both my axes :x and got pwned since I was unable to do anything during the time ( so switching weapons works? I couldn’t afai remember)
